A once-weekly injection is a strange thing to remember.
Daily medication has rhythm working for it. It rides along with coffee, brushing your teeth, going to bed. Miss it and the gap is obvious by lunchtime. A weekly dose has none of that. It has to survive seven days of ordinary life with nothing to attach itself to — and by day five, "was that Thursday or Friday?" is a genuinely hard question.
So: do reminders fix that? The honest answer has two halves, and the second one is the interesting part.
Quick answer
Reminders do help. Across chronic diseases, in randomized trials, text-message and app-based reminders produce small-to-moderate improvements in medication adherence — real, replicated, and considerably less dramatic than the marketing around them.
And then the gap: not one of those trials was done in people taking GLP-1 medication. Every number below comes from diabetes, hypertension, HIV, asthma, and similar conditions — nearly all of them daily oral regimens. The evidence base for the exact problem you have does not exist yet.
That doesn't make reminders a bad idea. It makes them a reasonable idea with borrowed evidence, which is a different thing from a proven one.
What the trials actually found
Three meta-analyses carry most of the weight here.
Thakkar et al. (2016), published in JAMA Internal Medicine, pooled randomized trials of SMS text-message reminders across chronic diseases. Text reminders roughly doubled the odds of medication adherence. In absolute terms, adherence went from about 50% to about 67.8% — a gain of 17.8 percentage points.
That is the largest effect in this article, and it is worth reading carefully. Doubling the odds sounds enormous. What it meant in practice was moving a group from coin-flip adherence to roughly two-thirds adherence. Better. Not solved.
Peng et al. (2020), in the Journal of Managed Care & Specialty Pharmacy, pooled 14 randomized trials of mobile apps covering 1,785 adults with chronic disease. Apps improved adherence with a Cohen's d of 0.40 (95% CI 0.27–0.52) — a small-to-moderate effect. The authors graded the quality of the underlying evidence as low, which is their own assessment, not a critic's.
Belete et al. (2023), in BMC Endocrine Disorders, narrowed to type 2 diabetes: 9 randomized trials, 1,121 participants. SMS reminders improved adherence with a standardized mean difference of 0.36 (95% CI 0.14–0.59). One detail is more useful than the headline: the effect was stronger in interventions that ran six months or longer. Whatever reminders do, they did not do it in a two-week burst.
Three different research groups, three different methods, three answers that land in the same modest range. That consistency is the strongest thing about this evidence — stronger than any single number in it.
What none of it tested
Being precise about the limits matters more than the effect sizes:
- No GLP-1 population. No randomized trial has tested reminder or app interventions on GLP-1 adherence specifically. Weekly injectable dosing was not the setting for any of this work.
- The outcome was adherence, not health. These analyses measured whether people took their medication, not whether they ended up healthier. Improved adherence is a plausible route to better outcomes, not a demonstration of one.
- Weekly is a different problem from daily. Almost everything here concerns regimens taken every day. A daily reminder competes with a habit that already half-exists. A weekly reminder has to work cold, seven days after the last one, often on a day that looks like every other day.
- Low-quality evidence, by the authors' own grading. Peng's GRADE rating was low. Take the direction seriously and the decimal places less so.
The closest thing to GLP-1 evidence — and it isn't a trial
There is one adjacent signal, and it needs its label attached.
Talay et al. (2025) looked back at 19,693 people using a tirzepatide digital service in the UK. In their analysis, consistent weekly self-weighing was the strongest predictor of whether someone was still adherent at 12 months.
This is an observational cohort. Nobody was randomly assigned to weigh themselves. People who step on a scale every week are plausibly different from people who don't — more settled, fewer side effects, better circumstances, more able to keep paying for treatment. Any of those could produce the association without the weighing doing the work.
So it is not evidence that a weekly ritual keeps you adherent. It is evidence that a weekly ritual and adherence travel together — which, combined with the trial data above, is enough to make the idea worth building around. Not enough to promise anything.

Designing a reminder system that survives a real week
This section is practical reasoning, not trial findings — the studies above tested whether reminders work, not which reminder design works best. Read it as a sensible way to apply a modest effect, and adjust it to your life.
1. Anchor the dose to something that already happens weekly. A notification is a nudge; an anchor is context. Sunday grocery run, Monday morning standup, Friday laundry. The reminder becomes the second thing you notice, not the only thing.
2. Give the reminder somewhere to land. The failure mode of a weekly reminder is dismissing it in a moment when you can't act — driving, mid-meeting, half-asleep. Set it for a window when you're actually near your medication, not the moment you technically wake up.
3. Add one backup, not five. A single follow-up a few hours later catches the dismissed-in-the-car case. A cascade of notifications teaches you to swipe them away without reading, which is worse than having one.
4. Close the loop by recording it. The reminder tells you to dose. The record tells you whether you did. Without the second half, the honest answer to "did I take it?" is a memory test, and by day five memory loses. This is the part Belete's six-month finding points at indirectly: what survived was the sustained system, not the alert.
5. Build for the disrupted week, not the good one. Travel, refill delays, and stressful stretches are where routines actually break — a topic with its own playbook in when routine breaks. Reminders don't help if the medication is in a fridge 400 km away. Refill timing needs its own reminder, well ahead of running out.
6. Treat a missed dose as a question for a professional, not a puzzle to solve. If you miss a dose or realize you've lost track of your schedule, contact your prescriber or pharmacist. What to do next depends on your medication, your dose, and how much time has passed — and that decision is theirs, not an article's and not an app's.
What this adds up to
Reminders are a low-cost intervention with a modest, repeatedly measured benefit in conditions that aren't yours. That is a reasonable basis for setting one up. It is not a basis for expecting the notification to carry the whole routine.
The pattern that shows up across all of it — the trials that ran longer, the cohort where weekly weighing tracked with staying on treatment — is that the thing doing the work looks less like an alert and more like a durable weekly ritual that the alert happens to start.
Where Velto fits
Velto is the record-keeping half of that loop. Reminders follow your actual treatment interval — anywhere from 3 to 30 days, since not every plan is a clean weekly one — and logging the dose takes a few seconds, so the answer to "did I take it?" is a fact rather than a recollection.
No app in the research above, Velto included, has been shown to improve GLP-1 adherence. What a tracker can do is make the reminder-and-record routine cheap enough to keep doing in a bad week.
