Should I log symptoms I think are unrelated to the medication?

Yes. Whether something is related is not usually obvious in the moment, and a log that only contains symptoms you already believed were treatment-related cannot help you find out otherwise.

Why does using consistent symptom names matter?

Because recurrence is more informative than any single entry, and recurrence is only visible if the same symptom is named the same way each time. "Queasy", "nausea", and "off stomach" logged interchangeably will never group into a pattern.

Is severity or duration more useful to record?

Both, as separate fields. A brief severe episode and a persistent mild one are clinically different situations, and collapsing them into a single score loses exactly the distinction that matters.
